1. Photographers Agreement

Once you have been approved as an HGA photographer. Please complete this form and return via email.

2. FFF Donor Form

Please complete the attached and return with donated items for Fotos, Frames & Fun.

3. Heart Gallery Social Work Job Description

Heart Gallery has a part-time social work position available. This position will assist in responding to inquiries and assist with in-state photoshoots.

5. Photo Shoot Referral Form

If you have are a DHR caseworker and you have a child that needs to be photographed please complete the above form and return to us. Once we have 5-7 children available we will come to your county for a photo shoot.
